ABSTRACT

Embora a avaliação da viabilidade miocárdica seja comum na prática do cardiologista, muitos médicos têm dúvidas a respeito dos resultados dos métodos diagnósticos. A medicina nuclear tem papel importante nos estudos de viabilidade, mas os laudos precisam ser interpretados num contexto clínico e fisiopatológico. Este artigo teve o objetivo de revisar a origem e a evolução do conceito da viabilidade miocárdica. São expostos os métodos diagnósticos com ênfase na medicina nuclear com uma explicação funcional sobre cada tipo de exame. A partir disso, são mostradas imagens como exemplos e é proposta uma maneira de atuar nesses casos baseada na clínica, na porcentagem de miocárdio acometido e na topografia das lesões coronarianas (proximais ou distais). Although assessing myocardial viability is a common cardiology practice, many physicians question the results of diagnostic methods. Nuclear medicine plays an important role in viability studies, but the reports require interpretation in a clinical and pathophysiological context. this article was aimed at reviewing the origin and evolution of myocardial viability. Here we present diagnostic methods by emphasizing nuclear medicine and provide a functional explanation of each test type using example images. We also propose how to act in these cases based on clinic examination findings, the percentage of affected myocardium, and coronary lesion topography (proximal or distal).(AU)

ABSTRACT

Resumen Nicotiana glauca también llamada Palán Palán, es un arbusto con hojas verdes azuladas y despulidas y una flor amarilla tubular pendulante que presenta alcaloides piridínicos, como nicotina, nornicotina, anatabina y anabastina (análogo estructural de la Nicotina). Se presenta el caso clínico de una paciente de 50 años con cuadro agudo de debilidad muscular generalizada que evoluciona con paro respiratorio, tras la ingesta accidental de una cantidad desconocida de hojas de Nicotiana glauca, cultivadas en una huerta hogareña mediante técnica de hidroponía y confundidas por su conviviente con espinaca. Presentó aumento de lactato y Troponina Ultra Sensible e Hipoquinesia Global de Ventrículo Izquierdo en el ecocardiograma, compatible con Aton tamiento Miocárdico (AM), que evolucionó favorablemente. Si bien hay pocos reportes, se han informado muertes de animales y humanos, tras la ingesta accidental de Nicotiana glauca. El inicio del cuadro es rápido, con patrón bifásico, con vómitos y estímulo simpático, seguido por bloqueo ganglionar y neuromuscular, pudiendo presentar paro respiratorio, shock, convulsiones y coma. El AM es una disfunción miocárdica prolongada con retorno gradual de la actividad contráctil, posterior a un episodio breve de isquemia grave, puede ser asintomático, pudiendo presentar alteraciones en el electrocardiograma, enzimas cardíacas o ecocardiograma. Generalmente presenta pronóstico favorable, pudiendo presentar insuficiencia cardíaca ante patologías concurrentes o aumento de requerimientos de oxígeno.


Abstract Nicotiana glauca is a shrub with bluish green leaves and a pendulous tubular yellow flower. It has pyridine alkaloids, such as nicotine, nornicotine, anatabine and anabastine (structural analog of Nicotine). We present the case of a 50 years old pa- tient with acute generalized muscle weakness that evolves to respiratory arrest, after accidentally ingesting an unknown quantity of Nicotiana glauca leaves, grown in a home vegetable garden, using a hydroponic technique and confused by her cohabiting with spinach. She presented increased lactate and Ultra Sensitive Troponin and Left Ventricular Global Hypokinesia in the echo- cardiogram, compatible with Myocardial Stunned, that it evolved favorably. There are few reports, animal and human deaths have been reported following accidental ingestion of Nicotiana glauca. The onset of the symptoms is early, with a biphasic pattern, with vomiting and sympathetic stimulation, followed by ganglionic and neuromuscular blockage and may present respiratory arrest, shock, seizures and coma. Myocardial Stunned is a prolonged myocardial dysfunction with gradual return of contractile activity after a brief episode of severe ischemia, it can be asymptomatic, and it can present alterations in the electrocardiogram, cardiac enzymes or echocardiogram. Generally presents a benign prognosis, being able to present heart failure with concurrent patholo- gies or increased requirements.

ABSTRACT

Contexto: la fibrilación y el flutter auricular son arritmias frecuentes en pacientes con miocardiopatía hipertrófica e incrementan su morbilidad. La cardioversión de la arritmia mejora la calidad de vida del paciente, sin embargo, no existe información sobre la eficacia o predictores de esta terapia en la literatura. Objetivo: identificar en pacientes que presentan miocardiopatía hipertrófica asociada a fibrilación o flutter auricular que fueron sometidos a cardioversión eléctrica, las variables relacionadas con la evolución de la arritmia después de esta terapia. Métodos: se analizaron los datos de 37 pacientes con diagnóstico de miocardiopatía hipertrófica asociada a fibrilación auricular (n=21) y flutter auricular (n=16), sometidos a cardioversión eléctrica, en lo relativo a éxito inmediato (reversión después del choque), recurrencia (reaparición del problema después de la cardioversión eléctrica) y éxito tardío (ritmo sinusal observado en la última consulta registrada). Resultados: el éxito inmediato se produjo en el 96% de procedimientos de cardioversión eléctrica en pacientes con fibrilación auricular y en el 100% de pacientes con flutter auricular. La recurrencia se produjo en el 76,9% de los pacientes con fibrilación auricular sometidos a cardioversión eléctrica después de un tiempo medio de 630 días y en el 57,1% de los pacientes con flutter auricular luego de un seguimiento de 1.138 días. Conclusión: se evaluó la eficacia de la cardioversión eléctrica mediante los resultados clínicos observados en pacientes con miocardiopatía hipertrófica asociada a fibrilación o flutter auricular; además se identificaron las variables relacionadas con la recurrencia y el éxito tardío después de la cardioversión eléctrica. Background: fibrillation and atrial flutter are frequent arrhythmias in patients with hypertrophic cardiomyopathy and increase their morbidity. Cardioversion of arrhythmia improves the quality of life of the patient; however, there is no information on the efficacy or predictors of this therapy in the literature. Aim: to identify the variables related to the evolution of the arrhythmia after therapy in patients with hypertrophic cardiomyopathy associated with atrial fibrillation or flutter were underwent electrical cardioversion. Methods: data from 37 patients with atrophic cardiomyopathy associated with atrial fibrillation (n = 21) and atrial flutter (n = 16), submitted to electrical cardioversion, were analyzed for immediate success (reversion after shock), Recurrence (recurrence of the problem after electrical cardioversion) and late success (sinus rhythm observed at the last recorded visit). Abstract Background: Atrial fibrillation (AF) is the most common arrhythmia seen in adults. Atrial stunning is defined as the temporary mechanical dysfunction of the atrial appendage developing after AF has returned to sinus rhythm (SR). Objectives: We aimed to evaluate atrial contractile functions by strain and strain rate in patients with AF, following pharmacological and electrical cardioversion and to compare it with conventional methods. Methods: This study included 41 patients with persistent AF and 35 age-matched control cases with SR. All the AF patients included in the study had transthoracic and transesophageal echocardiography performed before and after. Septum (SEPsSR), left atrium (LAsSR) and right atrium peak systolic strain rate (RAsSR) were defined as the maximum negative value during atrial contraction and septum (SEPε), left atrium (LAε) and right atrium peak systolic strain (RAε) was defined as the percentage of change. Parameters of two groups were compared. Results: In the AF group, 1st hour and 24th hour LAε, RAε, SEPε, LAsSR, RAsSR, SEPsSR found to be significantly lower than in the control group (LAε: 2.61%±0.13, 3.06%±0.19 vs 6.45%±0.27, p<0.0001; RAε: 4.03%±0.38, 4.50%±0.47 vs 10.12%±0.64, p<0.0001; SEPε: 3.0%±0.22, 3.19%±0.15 vs 6.23%±0.49, p<0.0001; LAsSR: 0.61±0.04s-1, 0.75±0.04s- 1 vs 1.35±0.04s-1, p<0.0001; RAsSR: 1.13±0.06s-1, 1.23±0.07s-1 vs 2.10±0.08s- 1, p<0.0001; SEPsSR: 0.76±0.04s- 1, 0.78±0.04s- 1 vs 1.42±0.06 s- 1, p<0.0001). Conclusion: Atrial strain and strain rate parameters are superior to conventional echocardiographic parameters for the evaluation of atrial stunning in AF cases where SR has been achieved.


Resumo Fundamento: A fibrilação atrial (FA) é a arritmia mais comum em adultos. Define-se atordoamento atrial como a disfunção mecânica temporária do apêndice atrial que se desenvolve depois de reversão da FA ao ritmo sinusal (RS). Objetivos: Avaliar as funções atriais contráteis através de strain atrial e strain rate em pacientes com FA, após cardioversão farmacológica e elétrica, assim como compará-los com os métodos convencionais. Métodos: Este estudo incluiu 41 pacientes com FA persistente e 35 controles com RS e pareados por idade. Todos os pacientes com FA incluídos neste estudo foram submetidos a ecocardiografia transtorácica e transesofágica antes e após. Strain rates de pico sistólico do septo (SEPsSR), do átrio esquerdo (LAsSR) e do átrio direito (RAsSR) foram definidas como o máximo valor negativo durante contração atrial. Strains de pico sistólico do septo (SEPε), do átrio esquerdo (LAε) e do átrio direito (RAε) foram definidas como porcentagem de mudança. Resultados: No grupo com FA, os parâmetros LAε, RAε, SEPε, LAsSR, RAsSR e SEPsSR da 1a hora e da 24a hora foram significativamente mais baixos que no grupo controle (LAε: 2,61%±0,13; 3,06%±0,19 vs 6,45%±0,27; p<0,0001; RAε: 4,03%±0,38; 4,50%±0,47 vs 10,12%±0,64; p<0,0001; SEPε: 3,0%±0,22; 3,19%±0,15 vs 6,23%±0,49; p<0,0001; LAsSR: 0,61±0,04s-1; 0,75±0,04s-1 vs 1,35±0,04s-1; p<0,0001; RAsSR: 1,13±0,06s-1; 1,23±0,07s-1 vs 2,10±0,08s-1; p<0,0001; SEPsSR: 0,76±0,04s-1; 0,78±0,04s-1 vs 1,42±0,06 s-1; p<0,0001). ABSTRACT

La miocardiopatía de takotsubo es una entidad que clínicamente simula un síndrome coronario agudo, siendo indispensable para su diagnóstico la exclusión de la enfermedad coronaria significativa. Se presenta el caso de una mujer postmenopáusica, quien durante un episodio de migraña desarrolla un cuadro clínico similar a un infarto del miocardio anterior, donde la ecocardiografía durante el evento agudo, orientó hacia el diagnóstico correcto de miocardiopatía de Takotsubo, encontrándose incidentalmente una arteria coronaria única sin enfermedad obstructiva.


Takotsubo cardiomyopathy is a condition that clinically simulates an acute coronary syndrome, thus it remains indispensable for its diagnosis to exclude a significant coronary disease. We present the case of a postmenopausal woman who developed a clinical picture similar to an anterior myocardial infarction during a migraine episode, where the echocardiogram during the acute event oriented towards the correct diagnosis of a Takotsubo cardiomyopathy, incidentally finding a single coronary artery with no obstructive disease.

ABSTRACT

BACKGROUND: Incidence of diastolic dyssynchrony (DD) and its impact on functional recovery of left ventricle (LV) after ST segment elevation myocardial infarction (STEMI) is not known. METHODS: Consecutive patients with STEMI who underwent successful revascularization were prospectively enrolled. Echocardiography with tissue Doppler imaging was performed within 48 hours of admission and at 6 months. LV end-diastolic volume index (EDVI), end-systolic volume index (ESVI), ejection fraction (EF), and left atrial volume index (LAVI) were calculated. Diastolic delay was calculated from onset of QRS complex to peak of E wave in tissue Doppler image and presented as maximal temporal difference between peak early diastolic velocity of 6 basal segments of LV (TeDiff). Study patients were compared with demographically matched control group. RESULTS: Forty eight consecutive patients (55 ± 10 years, 88% male) and 24 controls (56 ± 6 years, 88% male) were included. TeDiff was higher in STEMI than in controls (35.9 ± 19.9 ms vs. 26.3 ± 6.8 ms, p = 0.025). Presence of DD was higher in STEMI than controls (58% vs. 33%, p = 0.046) according to calculated cut-off value (≥ 29 ms). There was no correlation between TeDiff and change in EDVI, ESVI, and LAVI at 6 months, however TeDiff and change in EF at 6 months was positively correlated (r = 0.328, p = 0.023). Patients with baseline DD experienced remodeling less frequently compared to patients without baseline DD (11% vs. 38%, p = 0.040) during follow-up. CONCLUSION: STEMI disrupts diastolic synchronicity of LV. However, DD during acute phase of STEMI is associated with better recovery of LV thereafter. This suggests that DD is associated with peri-infarct stunned myocardium that is salvaged with primary intervention as well as infarct size.

ABSTRACT

Coronary artery disease (CAD) is one of the leading causes of death. Safflower attracts great attention owing to its anti-ischemia/reperfusion injury effect. Ninety-three patients with CAD were included and randomized into safflower treatment group, PCI group and control group. Low-dose dobutamine stress echocardiography (DSE) was performed to measure end-systolic volume (ESV), end-diastolic volume (EDV), left ventricular ejection fraction (LVEF) and wall motion score index (WMSI) to determine the recovery of hibernating myocardium and cardiac function in all patients before treatment and after 3-month follow-up. The study was to investigate the effects of safflower on hibernating myocardial revascularization and cardiac function. It was found that LVEF was significantly improved, while the ESV and WMSI were significantly reduced after 2-week treatment in safflower and PCI treatment groups. No significant differences were found between safflower and PCI treatment groups in ESV, EDV, WMSI and LVEF after treatment Safflower injection effectively improved hibernating myocardial function.

ABSTRACT

BACKGROUND: The purpose of this study was to evaluate whether performing an assessment of myocardial deformation using speckle tracking imaging during the recovery period after dobutamine stress echocardiography (DSE) allows detection of significant coronary artery disease (CAD) in patients with chest discomfort. METHODS: DSE and coronary angiography were performed in 44 patients with chest discomfort. The mean global longitudinal peak systolic strain (GLS) was measured at rest, at low stress (dobutamine infusion rate of 10 microg/kg/min) and at recovery (5 min after cessation of dobutamine infusion) of DSE using automated function imaging with apical views. Fractional flow reserve (FFR) was also performed in patients with intermediate coronary stenosis. CAD was defined as having a > or = 70% diameter stenosis on coronary angiography or as having a FFR < 0.8. Patients were divided two groups based on the absence or presence of CAD [CAD (-) group vs. CAD (+) group]. RESULTS: There were no significant differences in the clinical characteristics and results of conventional echocardiography between the two groups. GLS at recovery was lower in the CAD (+) group than in the CAD (-) group (-18.0 +/- 3.4% vs. -21.0 +/- 1.9%, p = 0.003). The optimal cutoff of GLS at recovery for detection of CAD was -19% (sensitivity of 70.6%, specificity of 83.3%). CONCLUSION: Assessment of GLS at recovery of DSE is a reliable and objective method for detection of CAD. This finding may suggest that systolic myocardial stunning remains even after recovery of wall motion abnormalities in patients with CAD.

ABSTRACT

A pesquisa de viabilidade miocárdica permite identificar segmentos disfuncionantes causados por obstruções arteriais que têm a capacidade de recobrar parcial ou totalmente a capacidade contrátil. Estima-se que 20% a mais de 50% dos pac ientes com disfunção crônica do ventriculo esquerdo t6êm significativa quantidade de miocárdio viável e, portanto, com potencial recuperação de contratilidade miocárdica após revascularização. Várias dfiferentes técnicas podem ser utilizadas para a identificação de miocárdio viável. Estudos demonstraram melhora acima de 5% da FEVE após revascularização em torno de 60% dos pacientes. Uma metanálise demonstrou associação significativa entre revascularização e melhora nas taxas de sobrevida em pacientes com evidência de viabilidade miocárdica, independente da técnica de imagem utilizada. Uma série de limitações dos estudos prévios de viabilidade deve ser considerada. Estes esteudis envolvem amostras com pequeno número de pacientes, são retrospectivos, ocorre uma inequívoca seleção seleção de pacientes e um bias de referência pós-teste. Foi recentemente publicado o Estudo STICH viability, estudo randomizado entre tratamento clínico e cirúrgico. Pacientes portadores de viabilidade miocárdica aprsentaram menores taxas de mortalidade, assim como menores taxas de do evento combinado de óbito e hospitalização por causas cardovasculares. Entretanto, após ajuste para outras variáveis de risco de um modelo multivariado, apresença de viabilidade não foi significativamente associada com a taxa de óbito, e sim a FEVE, volumes diastólicos e sistólicos finais. A viabilidade não deve, portanto, ser considerada obrigatória para a indicação cirúrgica, mas é p elevado e anatomia mais complexa.(AU)articularmente importante naqueles pacientes com risco.

ABSTRACT

A viabilidade miocárdica pode ser avaliada por testes de imagem não invasivos. Recentemente, a técnica de realce tardio miocárdico por ressonância magnética cardíaca se juntou a esta família de testes e é considerada, hoje, como a técnica padrão-ouro pela diretrizes nacionais e internacionais e pelo consenso de especialistas. Em medicina, não há palavra final; porém, no momento e baseado na literatura atual, a ressonância magnética cardíaca pode ser considerada pelo clínico como o mais próximo da palavra final em termos de viabilidade miocárdica para a decisão de revascularização miocárdica. As evidências atuais demonstram que a ressonância é um método capaz de definir áreas de miocárdio viável e não viável até nível quase microscópico. A correlação da viabilidade celular com a viabilidade contrátil, isto é, a probabilidade de melhora ou recuperação contrátil após revascularização miocárdica pode se predita com alta acurácia pela técnica de realce tardio miocárdio por ressonância magnética. Nos casos complexos, pode-se, ainda, aumentar a especificidade, associando-se a técnica de cinerressonância com dobutamina. Uma vasta gama de informações adicionais, como a detecção da obstrução microvascular ou no-reflow, entre muitas outras, conferem uma impressionante capacidade diagnóstica e prognóstica à ressonância magnética cardíaca na doença arterial coronária aguda e crônica. Os dados controversos do recente subestudo de viabilidade do estido STICH não responderam de forma definitiva às questões clínicas essenciais, que permanecem em aberto e, em especial, levantaram a hipótese de que a viabilidade deva ser investigada de rotina com métodos mais acurados, como a ressonância magnética cardíaca.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate cardiac function and myocardial perfusion during 48 h after cardiopulmonary resuscitation (CPR), further to test myocardial stunning and seek indicators for long-term survival after CPR.</p><p><b>METHODS</b>After 4 min of untreated ventricular fibrillation, fifteen anesthetized pigs were studied at baseline and 2 h, 4 h, 24 h, and 48 h after restoration of spontaneous circulation (ROSC). Hemodynamic data, echocardiography and gated-single photon emission computed tomography myocardial perfusion images were carried out.</p><p><b>RESULTS</b>Mean arterial pressure (MAP), coronary perfusion pressure (CPP) and cardiac troponin I (CTNI) showed significant differences between eventual survival animals and non-survival animals at 4 h after ROSC (109.2 ± 10.7 mmHg vs. 94.8 ± 12.3 mmHg, P=0.048; 100.8 ± 6.9 mmHg vs. 84.4±12.6 mmHg, P=0.011; 1.60 ± 0.13 ug/L vs. 1.75 ± 0.10 ug/L, P=0.046). Mitral valve early-to-late diastolic peak velocity ratio, mitral valve deceleration time recovered 24 h; ejection faction and the summed rest score recovered 48 h after ROSC.</p><p><b>CONCLUSION</b>Cardiac systolic and early active relaxation dysfunctions were reversible within survival animals; cardiac stunning might be potentially adaptive and protective after CPR. The recovery of MAP, CPP, and CTNI could be the indicators for long-term survival after CPR.</p>

ABSTRACT

We report a case of neurogenic cardiopulmonary instability with pulmonary edema occurring after an aneurysmal subarachnoid hemorrhage. The patient's pre-operative Glasgow coma scale score was 6 and the PA chest radiograph showed increased diffuse haziness in the right lung field. The patient presented with severe hypotension and low oxygen saturation during surgery. Cardiac damage was documented by increased CK-MB troponin-T levels, and ischemic ECG findings. Reversible cardiac failure associated with subarachnoid hemorrhage may be due to a neurogenic-stunned myocardium. The patient underwent clipping of the aneurysm and recovered with minimal neurologic impairment and normal cardiac function.

ABSTRACT

El empaquetamiento mediastinal estándar es una medida última frente al sangrado no controlable en cirugía cardíaca. Presentamos el caso de un paciente varón de 56 años de edad, con antecedente de estenosis aortica severa y aneurisma de aorta ascendente que fue sometido a cirugía de reemplazo de válvula aortica y aorta ascendente. Durante el transoperatorio presentó sangrado no controlable a través de las líneas de sutura aórtica­protésica y coagulopatía. Se implementó un empaquetamiento localizado de la aorta ascendente, como alternativa al empaquetamiento mediastinal clásico, con el objetivo de controlar el sangrado dejando libres de compresión las cámaras cardíacas derechas. No encontramos en la literatura procedimientos similares, se propone una alternativa para el manejo de estas complicaciones.


Standard mediastinal packing is an ultimate measure against non-bleeding. Controllable in cardiac surgery. We present the case of a 56-year-old male patient age, with a history of severe aortic stenosis and ascending aortic aneurysm that He underwent aortic valve replacement surgery and ascending aorta. During the transoperative presented uncontrollable bleeding through the suture lines aortic ­ prosthetic and coagulopathy. A localized packaging of the ascending aorta, as an alternative to classic mediastinal packaging, with the objective of controlling bleeding leaving the heart chambers free of compression right. We do not find similar procedures in the literature, we propose a Alternative for the management of these complications.

ABSTRACT

BACKGROUND: It was reported that N,N,N'N'-tetrakis-[2-pyridylmethyl]-ethylenediamine (TPEN), a transition metal chelator, confers cardioprotection against myocardial ischemic injury. In this study, we investigated the effect of TPEN targeting reperfusion period in isolated rat hearts. METHODS: Langendorff perfused rat hearts were subjected to 30 min of regional ischemia and 2 h of reperfusion. Hearts were randomly assigned to either control (n = 9) or 10 micrometer of TPEN (n = 8) groups. TPEN was perfused for a period of 5 min before and 30 min after reperfusion. RESULTS: The ratio of infarct area/ischemic area (AN/AR) was significantly reduced in TPEN treated hearts (6.9 +/- 1.7%, P 0.05). CONCLUSIONS: Pharmacological postconditioning with TPEN reduces myocardial infarction however, TPEN does not modify post-ischemic systolic dysfunction in isolated rat hearts.
